Use this short form if you have listed previous show(s) for the current show season for which the only changes are the show date, opening date, closing date, and judge.

Use this FADS SHORT SHOW FORM if you have already submitted a FADS LONG SHOW FORM for your first show of this show season and the only changes for this subsequent show are the dates and judge(s).
